Develop and document continuing care plans to support patient recovery and relapse prevention after discharge. Coordinate appointments and community resources while acting as a liaison between patients, counselors, and referral sources.
Requires an Associate's degree in nursing or human services, with a Bachelor's degree in social work or human services preferred. Current LPN/RN licensure and certifications in CPR, de-escalation, and restraint are required.
