The Care Coordination Specialist manages transitions of care, including insurance authorizations, payer communications, and utilization management. They are also responsible for managing referrals to external facilities and scheduling patient follow-up appointments.
A high school diploma or GED and experience in the healthcare industry are required. Proficiency in computer systems like EPIC and strong communication skills are essential, with a preference for 3 years of hospital experience.

The Care Coordination Specialist performs selected services and functions related Transitions of Care & Utilization Management, including insurance authorization, payer communications, Appeals and Denial information, and scheduling.
This is an onsite position, Monday - Friday 8:00-4:30pm which includes working every other Sunday.
