Provide psychosocial support and case management to individuals and families coping with acute and chronic illnesses. Responsibilities include developing intervention plans, coordinating care, and providing patient education and community referrals.
The candidate must be able to perform psychosocial assessments and formulate intervention plans for vulnerable populations. Proficiency in care coordination and supportive counselling is required.

Services include case management and interventions targeted at mitigating risk, promoting well-being and addressing barriers to healthcare access. This would be done through assessment and formulation of intervention plans, care coordination, providing supportive counselling, patient education, research and program development as well as providing information and referrals for other social services in the community.
