Job Duties
Consider joining the Virginia Department of Health as we foster healthy and resilient communities. This is your opportunity to serve the public in the Population Health Program at the Roanoke City/Alleghany Health District.
This is a restricted position, which is solely funded by the PHIG grant for period ending 11/30/2027. The availability of funding is scheduled for review and periodically thereafter. Continued employment is contingent on the continued availability of funds.
Job Description: The Population Health Community Coordinator serves as the Healthy Communities Coordinator for the local health district’s efforts to work on developing and implementing environmental strategies to improve population health. Represents the health district at community events and assists, as necessary, in the coordination of health community activities. Supports and reinforces healthy behaviors in the district by working with community coalitions, schools, businesses, city leadership, urban designers and other local organizations and partners to build support for lifestyle change
This coordinator position will help the health district meet community education and engagement goals for prevention and wellness with adolescents and young adults. The position will support partners through coalitions for LGBTQ+ engagement, substance use prevention, and reproductive and sexual health. The position will provide community education, plan and facilitate coalition meetings, and support outreach and engagement events.
Job Duties: Health Education & Promotion • Collects population health data to assess community needs and determine health education requirements. • Plans, implements, and evaluates health improvement programs and policies for employees and communities. • Provides guidance on chronic disease prevention and wellness promotion to staff and customers. • Identifies and implements opportunities for health improvement, including environmental and policy changes. • Coordinates health-related activities and develops partnerships to address health disparities. • Utilizes social media to promote health programs when appropriate. • Develops seminars and training sessions to enhance the department's public health expertise. • Responds to data requests.
Administrative & Resource Management • Procures supplies, including texts and materials. • Ensures compliance with HIPAA regulations for health department records. • May prepare grant applications, manage grant activities, and prepare reports. • Actively seeks funding, monitors budget, and ensures proper resource use.
Quality Improvement & Collaboration • Supports report writing, editing, and submission on goals and outcomes. • Verifies data accuracy, identifies trends, and prepares reports. • Develops work plans for health districts and partner agencies. • Acts as a liaison for local health districts on population health matters. • Promotes collaboration and consistency across health programs. • Assists in conducting community health assessments and improvement planning. • Collaborates with partner agencies to deliver health improvement opportunities. • Builds relationships, initiates projects, and seeks opportunities to improve health.
May be required to perform other duties as assigned. May be required to assist the agency or state government generally in the event of an emergency declaration by the Governor. May be required by VDH to adjust work hours, location, and job duties and participate in training to meet public health business needs, including emergencies. If this position is designated as a mobile worker, this includes both working in the field and teleworking at an alternate location when not in the field or during an emergency closing. You may be required to periodically come into your assigned office to attend meetings or training.
The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 35 local health districts.
Who We Are: The Roanoke City and Alleghany Health Districts (RCAHD) are two health districts in the Commonwealth of Virginia under the Virginia Department of Health and employ approximately 140 staff. RCAHD encompasses the cities of Covington, Roanoke, and Salem and the counties of Alleghany, Botetourt, Craig, and Roanoke.
What We Do: RCAHD, which provides services from nine office locations, serves a combined population of over 278,400 people throughout both rural and urban environments and works to promote and encourage healthy behavior, protect the public against environmental hazards, prevent epidemics and the spread of disease, respond to disasters, assist communities in recovery and assure the quality and accessibility of health services for all members of the communities.
The Virginia Department of Health offers 13 paid holidays, medical, dental, vision and life insurance, retirement plans (including 401a Cash Match and 457 Deferred Compensation Plan) as well as sick, family/personal, and annual leave.
VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.
Minimum Qualifications
• Knowledge of Health Education & Promotion: Understanding of population health data collection, community demographics, social/cultural environments, and chronic disease prevention. Ability to implement and evaluate health improvement programs and policies for diverse populations. • Communication Skills: Excellent oral and written communication skills to effectively guide staff, educate internal/external stakeholders, and respond to data requests. Ability to develop and deliver training materials, seminars, and health education sessions. • Resource Management: Knowledge of procurement processes, budget monitoring, and ensuring proper use of resources. Ability to prepare grant applications, manage grant activities, and maintain compliance with funding guidelines. • Collaboration & Relationship Building: Ability to build and maintain effective working relationships with community partners, local health districts, and agencies to promote health initiatives and improve population health. • Quality Improvement & Data Analysis: Skilled in analyzing and verifying data, identifying trends, and preparing reports to support health program planning. Ability to evaluate the success of health programs and make necessary adjustments. • Technology Skills: Proficiency in using social media platforms for health communication, as well as experience with data management systems and office software (Word, Excel, PowerPoint). • Project Management: Ability to develop and manage project plans, timelines, and resources to ensure successful implementation of health initiatives
