Conduct comprehensive medication reviews for high-risk Medicare members to improve drug therapy safety and effectiveness. Collaborate with multidisciplinary teams to address gaps in care and support STAR-related pharmacy initiatives.
Requires a PharmD degree and an active, unrestricted Pharmacist license in the state of residence. Preference is given to candidates with 3+ years of experience in Managed Care, MTM programs, and bilingual proficiency in English and Spanish.

Active and unrestricted Pharmacist license in the State of residence required
Strong clinical medication knowledge with the ability to apply evidence-based guidelines in practice
Minimum 3 years’ experience in Managed Care, Medicare and/or Medicaid settings preferred
Experience with Medication Therapy Management (MTM) programs strongly preferred
Familiarity with MTM platforms (ex: Adhere Health’s Optimize or similar applications) preferred
Excellent verbal and written communication skills, with the ability to effectively engage diverse audiences
Bilingual proficiency in English and Spanish
Advanced proficiency in Microsoft Office applications including Word, Excel, PowerPoint, and Outlook
Strong computer literacy with the ability to navigate systems and applications simultaneously
Strong learning agility and ability to apply new concepts and tools
Strong analytical thinking, problem solving and clinical decision-making skills
Ability to develop innovative solutions to complex healthcare challenges
PREFERRED JOB REQUIREMENTS:
Strong experience with MTM required
Bilingual (English and Spanish preferred)
Must be willing to work Phone Queue
Must be clinically strong with experience in diverse programs (MTM, Case Referrals, Narcotics)
*Residency and/or Board Certification preferred
