The Licensed Practical Nurse will collaborate with RNs and physicians to provide direct patient care, including medication reconciliation and specialized procedures. They are responsible for monitoring patient conditions, teaching self-care techniques, and maintaining accurate clinical documentation.
Candidates must possess a current LPN license and at least one year of clinical experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
Verify plan of care with the RN prior to each visit.
