The Case Manager will conduct community outreach to homeless and at-risk veterans to assess their needs and provide rapid re-housing or homelessness prevention services. They are responsible for developing housing stability plans, coordinating community resources, and maintaining accurate documentation in the HMIS database.
Candidates should have college-level coursework in human services or a related field and experience working with veterans or vulnerable populations. A valid Ohio driver's license and the ability to pass a criminal background check are required.

SUPERVISOR: Program Manager
The Duties and Responsibilities above represent the most significant duties of this position, but do not exclude other assignments that would be within the qualification and responsibility levels of this position.
Requirements
MINIMUM QUALIFICATIONS:
COMPLIANCE:
