The nurse will coordinate total patient care and evaluate the effectiveness of nursing services for patients and their families. Responsibilities include performing admissions, transfers, recertifications, and discharge OASIS assessments for home care patients.
Candidates must be a graduate of an accredited School of Nursing and hold a current Registered Nurse license in the state. At least one year of experience in a community or home health agency or hospital is preferred, along with a valid driver's license and current CPR (BLS) certification.
