The Supportive Housing Specialist assists tenants in securing and maintaining stable, independent housing while monitoring their mental health and service linkages. They provide ongoing support, including transportation, entitlement assistance, and coordination with service providers to ensure a person-centered living environment.
Candidates must hold a bachelor's degree or have four years of relevant mental health experience, with one year in human services preferred. A clean NYS driving record and the use of a personal vehicle are mandatory requirements for the role.
