The Nurse Practitioner will conduct comprehensive adult and geriatric assessments for hospice patients and manage their individual plans of care. They are responsible for maintaining accurate documentation and ensuring continuity of care within a hospice setting.
Candidates must hold an active, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) license in the state of practice. Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
