Engage and support clients with severe mental illness and substance use disorders to enroll them in care models and achieve wellbeing goals. Coordinate referrals, manage a caseload of 60-100 members, and provide in-person community outreach and documentation.
Requires a Bachelor's degree in Social Work, Psychology, or related fields, or equivalent experience, with at least 3 years of direct community experience. Must have a valid driver's license, reliable transportation, and proficiency with mobile technology and Google Workspace.
