GENERAL DESCRIPTION
Performs moderately complex (journey-level) case work. Work involves serving as a liaison between clients and their families in obtaining assistance and rehabilitative treatment. Works under general supervision with limited latitude for the use of initiative and independent judgment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Interviews clients, their families, law enforcement officials, schools, and health and social welfare institutions to obtain client background information. Identifies the psychosocial, economic, and physical needs of clients. Assesses clients’ support systems and available community resources, schools, and facilities. Provides case management services in the process of client admission, discharge, or furlough. Counsels clients and their families and coordinates placements of clients with community facilities and schools. Provides Psychosocial rehabilitation skills training, skills training under IMR model medication training and support, crisis intervention services, supported Housing and Supported employment and other related services to individuals, families, and others as appropriate. Serves as liaison between clients and their families, state and judicial systems, and facility or treatment providers. Document case records, assessments, person centered recovery plans and progress notes according to Center requirements and Texas Administrative Codes. Employees will maintain complete and accurate consumer medical records and keep all such information confidential. Assists with developing individual treatment plans, and in monitoring the observance of clients’ rights. May participate in individual, group, or family therapy sessions. May participate as an active member of an interdisciplinary team. Meets and maintains all minimum service requirements according to Center requirements, Resilience Disease Management Design and Texas Administrative Codes Maintains department productivity, contract outcome, and documentation requirements. Participates in training and time studies for Medicaid Administrative Claiming (MAC). Performs related work as assigned. Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the 8-5 work day or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes. Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center. Frequent daily driving within the catchment area is required.
GENERAL QUALIFICATIONS
Experience/Education/Licensing/Certification
Experience in a field relating to the agency program. Graduation from an accredited four-year college or university with a major course work in a field relevant to the assignment. Experience and education may not be substituted for one another.
Knowledge/Skills/Abilities
