Position Summary:
The REUP215 Aftercare & Family Support Coordinator is responsible for providing intensive family engagement, aftercare case management, educational advocacy, and coordination of community resources for youth and families participating in REUP215.
This position serves as the bridge between diversion services, family stabilization, educational success, and long-term community engagement. The Coordinator works closely with youth, caregivers, schools, community partners, and REUP215 Diversion Coaches to strengthen family relationships, reduce barriers to success, increase community connectedness, and support sustained positive outcomes following enrollment in the diversion program.
The position also oversees and facilitates Guardian Empowerment League (GEL) activities and caregiver engagement initiatives. Responsibilities align with practices in family support, case management, community resource coordination, and juvenile justice aftercare. This role focuses on strengthening family relationships, reducing parent-child conflict, and increasing family stability to support successful youth reintegration.
The position emphasizes consistent engagement, resource connection, and structured family support interventions using a trauma-informed, culturally responsive, and strengths-based approach.

Conduct bi-monthly (twice per month) meetings with families to assess needs, provide support, and monitor progress.
Facilitate parent-child conflict resolution sessions to improve communication, trust, and relationship stability.
Provide coaching to caregivers on effective parenting strategies, behavior management, and youth engagement.
Identify and address barriers impacting family functioning, including stress, trauma, and system involvement.
Connect families to community-based resources, including:
Behavioral health services.
Financial and housing supports.
Parenting programs.
Educational and social services.
Plan and host Guardian Empowerment League twice per month (one in person and virtual).
Plan quarterly events for families.
Collaborate with Reintegration Specialists, case managers, and other team members to align family support with youth service plans.
Participate in case reviews and team meetings to provide updates on family progress.
Maintain consistent communication with families to encourage engagement and retention in services.
May include a caseload of up to 14 youth.
Document all contacts, services, and progress in accordance with the agency.
Report any safety concerns or critical incidents promptly.
Job Qualifications:
Minimum of 2 years of experience working with families and/or at-risk youth populations.
Experience working with families of justice-involved or high-risk youth.
Knowledge of family systems, trauma, and adolescent development.
Skills:
Strong skills in family engagement, conflict resolution, and mediation
Ability to build trust and rapport with caregivers and youth
Strong understanding of community resources and service systems
Ability to work collaboratively within a multidisciplinary team
Effective communication, organization, and documentation skills
Commitment to culturally responsive, strengths-based, and family-centered practice
Education Required:
