Housing Resources Case Coordinator

Job Description Summary:

The Housing Resources Case Coordinator is a senior-level HRC Specialist staff position responsible for preparing individuals and families experiencing homelessness for successful housing placement opportunities. This role focuses on ensuring participants are "housing document ready" by obtaining, organizing, and maintaining critical documentation required for housing programs, subsidies, and landlord applications.

Working within a Housing Readiness framework, the Housing Resources Case Coordinator proactively engages participants in shelters, community locations, partner agencies, and other community-based settings to collect and secure vital documents such as Social Security cards, birth certificates, state identification cards, income verification, disability documentation, and other supporting records needed for housing placement.

This position requires extensive community-based work, strong relationship-building skills, independent decision-making, and the ability to meet participants where they are in the community to remove barriers to housing. The Housing Resources Case Coordinator serves as a key liaison between participants, housing navigation staff, community partners, government agencies, and housing providers to ensure documentation is complete and readily available when housing opportunities arise. This role provides a higher level of participant engagement and case coordination than entry-level Housing Resources HRC Specialist positions.

Essential Functions

• Conduct comprehensive housing readiness assessments to identify documentation needs and barriers to housing placement.
• Meet program participants in shelters, encampments, community locations, partner agencies, hospitals, libraries, and other approved locations to assist with document collection and housing preparation.
• Assist participants in obtaining essential documents, including but not limited to: 
  • Social Security Cards
  • Birth Certificates
  • State Identification Cards
  • Driver's Licenses
  • Income Verification
  • Disability Documentation
  • Veteran Documentation
  • Proof of Residency
  • Benefit Award Letters
  • Banking Information
  • Other housing-required documentation
• Coordinate with government agencies, healthcare providers, schools, employers, and community partners to obtain necessary records.
• Maintain organized and secure participant files while ensuring confidentiality and compliance with all agency policies and funding requirements.
• Track document acquisition progress and maintain documentation readiness status for participants actively seeking housing.
• Collaborate with Housing Coordinators, Housing Navigators, Case Managers, and community partners to prepare participants for available housing opportunities.
• Assist participants with housing applications, subsidy applications, and landlord-required paperwork.
• Identify and address barriers that may delay housing placement, including missing identification, legal documentation issues, or benefit verification concerns.
• Advocate on behalf of participants with partner agencies and public systems to expedite document retrieval and housing placement processes.
• Provide strengths-based support and engagement services utilizing trauma-informed and person-centered practices.
• Educate participants on document retention, housing requirements, and tenancy readiness.
• Maintain accurate and timely documentation within HMIS and other required databases.
• Develop and maintain productive working relationships with landlords, housing providers, shelters, and community partners.
• Participate in case conferencing meetings, housing prioritization meetings, and coordinated entry activities as needed.
• Representing Community Housing Network and the Housing Resources Department at community meetings, outreach events, and collaborative initiatives.
• Continue professional development through training, conferences, and best practice updates related to housing stability and homelessness services.
• Perform other duties as assigned.

Qualifications and Skills: 

Education and Experience

• Associate's degree in Human Services, Social Work, Psychology, Public Administration, or related field preferred.
• Bachelor's degree in a human services-related field preferred.
• CHN Housing Resources Specialist call center experience.
• Minimum of three (3) years of experience in housing services, homelessness programs, case management, outreach services, benefits assistance, or related human service experience.
• Equivalent combinations of education, lived experience, and professional experience may be considered.

Knowledge, Skills, and Abilities

• Strong understanding of homelessness, housing barriers, and housing navigation processes.
• Experience assisting individuals in obtaining identification and eligibility documentation.
• Knowledge of community resources, public benefits, housing programs, and coordinated entry systems.
• Ability to build rapport with individuals experiencing homelessness and other vulnerable populations.
• Strong problem-solving and critical-thinking skills.
• Excellent verbal and written communication skills.
• Ability to work independently while maintaining regular communication with supervisors and team members.
• Strong organizational skills with attention to detail and documentation requirements.
• Proficiency in Microsoft Office Suite, Outlook, internet-based systems, and HMIS or similar databases.
• Ability to manage multiple participants and competing priorities in a fast-paced environment.
