Job Description Summary:

Working within a Housing Readiness framework, the Housing Resources Case Coordinator proactively engages participants in shelters, community locations, partner agencies, and other community-based settings to collect and secure vital documents such as Social Security cards, birth certificates, state identification cards, income verification, disability documentation, and other supporting records needed for housing placement.
This position requires extensive community-based work, strong relationship-building skills, independent decision-making, and the ability to meet participants where they are in the community to remove barriers to housing.
