The Social Worker provides high-quality case management and advocacy for persons with disabilities and their families. They conduct assessments, provide counseling, and coordinate comprehensive care between healthcare providers and community resources.
Candidates must hold a Bachelor or Master's Degree in Social Work and be registered or eligible for registration with the SASW. Experience working with special needs clients or in a similar setting is considered an advantage.

Bringing to each case an understanding of the biopsychosocial concerns of families and the influences of family, community and cultural differences, the Social Worker is responsible for providing high quality and effective case management. The Social Worker acts as a liaison between the client, healthcare providers, and community resources to coordinate comprehensive care and support services and advocates on behalf of clients to ensure they receive the necessary assistance to live as independently as possible.
Key Responsibilities
The social worker in our early intervention and special schools brings to the educational process an understanding of the psychosocial concerns of families and the influences of family, community and cultural differences as they interact with the educational process.
The social worker will conduct assessments of client needs, provide counselling, support and crisis intervention and participate in programme and policy development.
The social worker works closely with families, social and community agencies and volunteers to strengthen the circles of care around our clients with disabilities.

To serve adults with disabilities, Rainbow Centre is building a diverse range of programmes including the Disability Case Management Programme, a life coaching service for young adults designed to enhance inclusion, befriending and self-employment services for families.
