Provide clinical case management and therapeutic services for residents with serious mental health and substance use disorders using an Assertive Community Treatment model. Requires a high school diploma and at least 2 years of experience in substance recovery and working with chronically homeless populations or those with serious mental illness. A valid California Driver's License, personal vehicle, and CADACC I or II certification are preferred.

About The Role: The Health and Wellness Navigator will provide clinical/therapeutic services, with focus on substance use disorders, on an Assertive Community Treatment team. This team will do outreach, engage, house, and provide wrap around clinical services for all residents. The residents are referred to by the County Office based on county data and other assessment processes as being the most frequent users of emergency services, criminal justice, and other homeless/county services. Program participants will have Serious Mental Health Disorders, Substance Use Disorders, and/or Chronic Health Conditions.

Abode's mission is to end homelessness by assisting low-income, un-housed people, including those with special needs, to secure stable, supportive housing; and to be advocates for the removal of the causes of homelessness. We develop and implement innovative programs to end homelessness. Our agency is built on the principles of Housing First, a proven approach that has demonstrated results far superior to the traditional approaches to homelessness. We were founded in Alameda County in 1989 when a small group of people came together to address a growing problem with family homelessness in the Tri-City Area. Since then, we have expanded our impact throughout the Bay Area, serving more than 15,000 adults and children each year in Alameda, Santa Clara, San Francisco, San Mateo, Santa Cruz, Napa, and Solano counties. Our results show how effective our work has been in ending homelessness for vulnerable adults and children throughout our community.
