Provide direct case management, counseling, and advocacy to homeless families to facilitate their transition to permanent housing. Manage intake assessments and service plans using the CARES system while coordinating with various city agencies.
Requires at least an Associate's degree in Social Work or a related field and one year of experience in social or housing services. Candidates must have experience facilitating groups for homeless adults and knowledge of substance abuse treatment.
