Provides high quality, comprehensive, professional nursing primary nursing care to individuals and families throughout the lifespan. As part of a clinical team, the PCN works at the top of his or her licensure to enhance the safety, effectiveness, and efficiency of care through both the performance of professional nursing responsibilities and the delegation of technical and clerical functions as appropriate to medical assistants. Provides patients with both general and focused health education regarding preventive, chronic, and episodic health care issues. Assumes responsibility, in collaboration with primary care providers to coordinate care, support transitions between levels of care, and counsel, educate, and support patients in improving health status. Applies principles of advanced access, planned care and the chronic care model, and rapid cycle performance improvement using the PDSA approach at CHCI.
ROLE AND RESPONSIBILITIES
Support primary care of patients under planned care model, including pre-visit planning, daily huddles, and execution of nurse-led and nurse-assisted visits.
Collaborates with PCP to identify needs for preventive care, evidence based chronic disease care, and care coordination.
Provides general and specific health care education during patient visits and by telephone.
Works with patients to identify and monitor self-management goals.
Collaborates and coordinates care with other community based providers.
Follows up on emergency room visits and hospital discharges.
Assists providers with exams, treatments, and procedures as needed.
Ensures safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
Administers medications, including immunizations, at the order of prescribing provider and under standing orders as established by CHCI.
Adheres to CHC protocols regarding documentation of medication, route, dosage, and information provided to patient.
Monitors patient response to medication based on specific medication administered; detects, reports, and responds to any adverse reaction. Reconciles medications. Provides patient education as appropriate to the administered medication.
Performs wide range of nursing activities as needed and after initial prescription by primary care provider such as wound care, catheterization, etc.
Accurately documents all aspects of patient care in the medical chart on a timely basis.
Schedules tests and referrals for specialty care on order of provider.
Maintains strict infection control standards in accord with CHCI protocols and universal precautions TJC, OSHA and CLIA standards.
Acts as a resource person for medical assistants regarding patient preparation, needs and procedures needed.
Monitors and ensures follow up of all abnormal pap smear results, STDs, lead screens and other tests and screens performed at CHCI.
Oversees the maintenance of supplies and equipment, ordering supplies as necessary.
Maintains compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.)
Expert use of electronic health record to ensure timely, safe, confidential storage and retrieval of patient information.
Participates in Project ECHO complex care management to continually advance knowledge and skills in care management and care coordination.
Presents nursing information and insight into patient care with other members of the care team at Integrated Care Team meetings.

1+ year of experience as a nurse in ambulatory care, preferred. New graduates and experienced nurses from other settings with a strong interest in chronic disease management and primary care nursing are encouraged to apply.

CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certification.
LPN
On a daily basis, provides clinical and technical support for primary care providers. Implements all planned care items using electronic tools to ensure efficient and accurate workflow. Monitors equipment and performs clinical testing according to OSHA and CLIA standards. Liaises with patients, families, specialists, and all members of the health care team. With the primary care nurse and provider, ensures continuity and coordination of care in a patient centered medical home. Assists the primary care nurse as able with routine medication administration, immunizations and other provider clinic support supported by the scope of LPN licensure.
Apply evidence based guidelines from major regulatory and specialty groups, performs pre-visit preparation for all primary care patients scheduled to be seen
In collaboration with the primary care nurse and provider, “e-huddles” utilizing the Planned Care Dashboard (an electronic tool that summarizing all care that is due for a patient at the time of their visit) to ensure complete and thorough primary care visits.
Prepares patients to be seen by provider, using established criteria as appropriately determined by the nature of the visit (i.e., sick, PE, WCC, etc.).
Accurately documents chief complaint and results from evidence based screening tests.
Accurately performs and documents patient vital signs.
Prioritizes work responsibilities on a daily basis, decreasing waiting time. Collaborates with providers, clinical and clerical support staff to facilitate appropriate patient flow. Effectively works as a member of the health care team assisting other team members as needed, or requested. Appropriately schedules tests and referrals for specialty care in accordance to Provider orders in a timely manner.
Maintains a clean, orderly, safe working environment. Oversees maintenance of supplies and documents in medical logs as directed.
Setup and stock exam rooms and clean rooms at end of each day worked.
Adheres to all infection control standards, follows Universal Precautions and safety policies and procedures of the Agency. Maintains yearly CPR certification as required for all clinical employees.
Exhibits excellent customer service skills to external and internal customers at all times with a focus on quality of care and teamwork that focuses on the needs of the department and/or the business in conjunction with the full mission of the Agency.
Complies with Agency mandated in-service training workshops (EOC, Working Safely, Abuse, etc.).
Assists the primary care nurse with provider clinic support such as routine medication administration, immunizations, and other items as able. All medications and immunizations should be administered under the delegation of CHCI’s standing order, or Primary Care RN or Primary Care Provider delegation.
