The RN Case Manager leads a multidisciplinary team to develop and communicate patient care plans while ensuring seamless transitions between care settings. They perform daily patient rounds to monitor progress, coordinate with social work and utilization review, and ensure discharge objectives are met.
Candidates must be graduates of an accredited nursing program with a current Texas or NLC nursing license and Basic Life Support certification. A minimum of 2 years of experience in an acute care hospital setting is required.
