The Case Manager handles utilization reviews, resource management, and the development of safe, timely discharge plans for critically ill patients. They coordinate with payors for reimbursement and facilitate multi-disciplinary team meetings to manage patient care.
Candidates must hold a current license as a Nurse (RN/LPN), Respiratory Therapist, or Social Worker. Previous experience in an inpatient hospital setting dealing with acute or critical care patients is required.
Case Manager (PRN)

Availability to work a minimum of 2 shifts per month on an "as needed" basis - shifts would be scheduled on weekdays, from appoximately 8a.m.-5p.m.

The Case Manager is responsible for utilization reviews and resource management, discharge planning, treatment plan management and financial management, while also completing medical record documentation. You will report directly to the Director of Case Management and provide social work services, as necessary, per state guidelines.
