The Registered Nurse Case Manager will provide hospice care services to patients within the designated service area of Jackson, Vinton, Pike, and Ross Counties. The role involves managing patient care plans and participating in an on-call weekend rotation.
Candidates must hold a degree in nursing from an approved school and possess a valid Registered Nurse license. A minimum of two years of nursing experience is required, with preference given to those with a background in home health or hospice.
