Provides care management, psychosocial assessments, and treatment planning for patients in various medical specialty areas. Collaborates with multidisciplinary teams to develop discharge plans and provide expert crisis intervention and patient advocacy.
Requires a Master of Social Work (MSW) and licensure as an LCSW or LSW in Pennsylvania. Candidates must have at least 5 years of social work experience in a healthcare setting and obtain necessary criminal and child abuse clearances.
Provides care management, ongoing psychosocial assessment and treatment planning to patients within a variety of specialty areas in the medical setting per regulatory requirements. Collaborates with the multidisciplinary team, lending insight and expertise and is responsible to develop appropriate discharge plans and resources to enhance independence and community adjustment. Provides consultation in areas of expertise to hospital and social work staff.
