Our services include primary care house calls, urgent care visits, chronic disease management, post-hospitalization follow-ups, lab orders, mental health support, medication management, and annual Medicare wellness exams. We specialize in helping recently discharged patients recover safely at home while reducing hospital readmissions and improving overall health outcomes. Now accepting new patients and referrals.
