The Case Manager is responsible for managing a caseload including screening, intake, counseling, and crisis intervention for patients with substance use and mental health disorders. They coordinate care with community providers and perform quality assurance activities to ensure compliance with agency and governmental regulations.
An Associate's degree in a related human services field is required, while a Bachelor's degree is preferred. Candidates must have two years of experience working with behavioral health or substance abuse issues.
Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position and located at our Insights Outpatient Program.
Schedule: Flexible schedule within the clinic’s operating hours Monday–Thursday, 8:00am–8:30pm and Friday - Sunday 8:00am - 4:30pm some evening hours required
In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!
Pay: $24.00 -$26.00 per hour.
