The Discharge Planner is responsible for performing psychosocial assessments and establishing appropriate discharge plans for patients in an acute care hospital. They collaborate with nursing staff and act as patient advocates to ensure access to community resources and primary care follow-ups.
A Bachelor's degree in a related field is required. Two to three years of experience in social services and discharge planning is preferred.
Under the direction of the Director of Case Management, the Discharge Planner is responsible for establishing an appropriate discharge plan for patient’s in the acute care hospital setting.
REQUIRED:
Bachelor's Degree in related field
PREFERRED:
2-3 years experience in all aspects of social service and discharge planning preferred
