The Social Day Case Manager is responsible for conducting client intakes, assessments, and managing an assigned caseload of seniors. They also coordinate daily program operations, including meal service, activity planning, and maintaining accurate program documentation.
Candidates should have an associate degree and three to five years of related experience in human services. A valid NYS driver's license is required to meet insurance standards.

As a Social Day Case Manager, your role will be to provide overall support to the Social Day/Day Hub and Congregate Meal Site. You will be responsible for intake and ongoing assement of particpating clients,case management, coordinating with program staff to support daily program operations while helping ensure clients receive high quality, person-centered services.
