The Case Manager will guide families through Medicaid waiver programs and manage service authorizations and documentation. They are responsible for conducting regular home visits to ensure clients receive necessary care and support.
Candidates must have at least 2 years of experience supporting individuals with disabilities or the elderly. A valid driver's license, a reliable vehicle, and proficiency with digital tools are required.
