The Per Diem RN will conduct in-home visits to assess members, develop individualized care plans, and provide training to caregivers. They will also maintain clinical documentation and monitor member health status in collaboration with the AFC team.
Candidates must hold an active RN license in Massachusetts and possess an Associate or Bachelor's degree in Nursing. Reliable transportation is required to travel to member homes, along with proficiency in Microsoft Office applications.
