The Hospice Social Worker provides casework and social services to patients and families facing terminal illnesses. They are responsible for developing multidisciplinary collaboration and assisting patients with personal and environmental difficulties.
Candidates must hold a Master's degree in Social Work and possess a valid driver's license. Previous experience in hospice, home health, and social work is required, along with BLS certification within 90 days of hire.
The Hospice Social Worker - MSW performs a variety of casework and provides social work services to patients, families, and significant Must develop and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
