The Community Habilitation Specialist works directly with individuals with developmental disabilities to support their independence and community integration. Responsibilities include assisting with money management, socialization, healthy lifestyle skills, and transportation to appointments.
Candidates must possess a valid New York State driver's license and access to a personal vehicle. A high school diploma or equivalent is required, with preference given to those with one year of experience in a health or human services field.
As a Community Habilitation Specialist, you will work directly with individuals with developmental/learning disabilities, providing services to the people we support so they can live independently and thrive in their communities. This could include, but is not limited to:
Preferred: One year pertinent experience in health and human service related field and experience with individuals with disabilities
Minimum: High school diploma or equivalent
