About the Role:
The HCBS Program Coordinator plays a pivotal role in managing and overseeing Home and Community-Based Services (HCBS) programs to ensure that individuals receive high-quality, person-centered care in their homes and communities. This position involves coordinating with multiple stakeholders including service providers, clients, and regulatory agencies to facilitate seamless service delivery and compliance with federal and state guidelines. The coordinator will develop, implement, and monitor program policies and procedures to enhance service effectiveness and client satisfaction. Additionally, the role requires continuous evaluation of program outcomes and the identification of opportunities for improvement to meet evolving community needs. Ultimately, the HCBS Program Coordinator ensures that vulnerable populations have access to supportive services that promote independence and improve quality of life.
Minimum Qualifications:
Bachelor’s degree in social work, public health, healthcare administration, or a related field.
At least two years of experience working with HCBS programs or similar community-based service programs.
Strong knowledge of federal and state HCBS regulations and compliance requirements.
Excellent organizational and communication skills, both written and verbal.
Proficiency in data management and reporting tools.
