The Physical Therapist will provide direct patient care, evaluate functional needs, and design personalized care plans in a home health setting. They will also document patient progress, lead support staff, and ensure quality care standards are met.
Candidates must be graduates of an APTA-approved physical therapy program and hold a current, unrestricted PT license. A minimum of one year of healthcare experience and the ability to travel within the designated coverage area are required.
