The Nurse Case Manager serves as the administrative point of contact for psychological health programs, coordinating behavioral health initiatives and case management for Soldiers and dependents. Key duties include establishing treatment plans, monitoring patient progress, and collaborating with military leadership to ensure operational readiness.
Candidates must have an Associate Degree in Nursing and at least two years of experience as a registered nurse case manager within the last three years. A professional certification in case management (such as CCM, CDMS, or RN-NCM) and a valid state nursing license are required.

Summary:
The Psychological Health Program (PHP) is designed to promote mental fitness and personal wellness to support operational readiness through linkage to behavioral health treatment, consultation, critical incident response, and clinical case management (CCM). The Psychological Health Program (PHP) is a support service that will assist Soldiers and dependents to access the services required to maintain optimum mental fitness and readiness.

The salary range for this position $85,000.00 to $95,000.00 annually.
