Job Summary
The Clinical Pharmacy Specialist in the ED is responsible for planning, organizing, and optimizing specialized pharmaceutical care to improve patient outcomes in a defined area of clinical expertise. Provides direct patient care services, working collaboratively with interdisciplinary clinical teams to support acute and chronic disease management, medication therapy optimization, and safe transitions of care. Collaborates with pharmacy and unit leadership on staff training, clinical education, and the evaluation of pharmacy services across the continuum of care. Participates in formulary reviews, medication use evaluations, and multidisciplinary initiatives. Acts as a clinical resource, promotes high standards of care, and fosters effective partnerships across departments, with a strong emphasis on patient-centered service.
ESSENTIAL FUNCTIONS
Clinical Leadership & Patient Care: Leads and supports specialized pharmaceutical care within a defined area of clinical expertise. Monitors and optimizes medication therapy to improve patient outcomes across the continuum of care.
Education & Mentorship: Provides clinical education to pharmacists, residents, students, and other interdisciplinary staff. Develops training programs, precepts learners, and serves as a resource within the clinical team.
Protocol & Formulary Management: Develops, implements, and evaluates clinical protocols and formulary items. Participates in formulary reviews, medication use evaluations, and ensures appropriate documentation of clinical actions.
Interdisciplinary Collaboration: Collaborates with physicians, nurses, and other healthcare professionals to enhance medication processes, ensure safe practices, and support system-wide clinical initiatives.
Quality Improvement & Safety: Engages in quality assurance and improvement projects. Monitors safety events, supports compliance with best practices, and leads efforts to optimize clinical outcomes.
Communication: Represents pharmacy on committees and project teams.
Operational & Strategic Support: Assists with clinical scheduling, interviews, and onboarding of staff. Partners with other Clinical Specialists and pharmacy leadership to align services and address system-level needs.

Education Requirements
