The Clinical Pharmacy Specialist is responsible for optimizing pharmaceutical care and providing direct patient care services within interdisciplinary teams. They also lead staff training, clinical education, and participate in medication use evaluations and multidisciplinary initiatives.
Candidates must hold a Doctorate degree or an equivalent combination of education and experience. A valid Pennsylvania Pharmacy License is required within 90 days of hire.
The Clinical Pharmacy Specialist in the ED is responsible for planning, organizing, and optimizing specialized pharmaceutical care to improve patient outcomes in a defined area of clinical expertise. Provides direct patient care services, working collaboratively with interdisciplinary clinical teams to support acute and chronic disease management, medication therapy optimization, and safe transitions of care. Collaborates with pharmacy and unit leadership on staff training, clinical education, and the evaluation of pharmacy services across the continuum of care. Participates in formulary reviews, medication use evaluations, and multidisciplinary initiatives. Acts as a clinical resource, promotes high standards of care, and fosters effective partnerships across departments, with a strong emphasis on patient-centered service.
