The program focuses on patients with chronic conditions, including hypertension, type 2 diabetes, chronic heart disease, and chronic kidney disease, who are identified through the Epic electronic medical record (EMR) as being at risk for unsuccessful telehealth use because of limited device access, internet connectivity, or digital literacy. The THRIVE Digital Equity Program is located in the Crosstown Building at 801 Massachusetts Avenue, second floor.
JOB RESPONSIBILITIES:
Coordinate the day-to-day implementation and operations of the THRIVE Digital Equity Program including patient identification, outreach, enrollment/participation, follow-up, and tracking across program activities.

Coordinate the pilot implementation of the Digital Health Literacy Course and assist with identifying and implementing improvements based on participant feedback.
Develop and maintain databases, tracking systems, and documentation for program participants and activities.
Monitor program metrics, including patient outreach, telehealth connection/completion, MyChart activation and navigation, digital literacy training participation, referrals, device distribution, and other program outcomes.
Assist with data analysis and interpretation to evaluate the effectiveness and impact of the program. Prepare routine reports, dashboards, summaries, and other materials for the Principal Investigator, program leadership, and grant reporting.
Identify trends and operational barriers and make recommendations for improving program efficiency and participant outcomes.
Coordinate referrals to community-based organizations and digital equity resources and maintain documentation of referral outcomes.
Maintain relationships and communication with community partners involved in digital literacy, technology access, internet connectivity, and other support services.
Assist with coordination of laptops/tablets and other technology resources provided to eligible participants.
Support expansion of digital health navigation services to additional BMC patient populations. Develop and maintain standardized workflows, training materials, and program procedures.
Collaborate with BMC clinical, research, IT, THRIVE, and administrative teams to integrate digital health navigation into existing workflows.
Ensure program activities are conducted in accordance with BMC policies, HIPAA/privacy requirements, and applicable research/IRB requirements, when applicable.
Participate in quality improvement activities designed to increase patient engagement, digital literacy, MyChart use, and successful telehealth completion.
Assist the PI and program leadership with identifying opportunities for program sustainability, scalability, and future funding.
