Requires a current Registered Nurse (RN) license, CPR certification, and a valid driver's license with reliable transportation. A minimum of 2 years of hospice nursing experience and familiarity with HomeCare HomeBase are preferred.
Choice Hospice is seeking an experienced Hospice RN Evaluator/Admission Nurse to provide compassionate, patient-centered care to individuals with life-limiting illnesses. In this role, you will complete hospice evaluations and admissions, determine eligibility, develop individualized plans of care, and educate patients and families while ensuring a smooth transition into hospice services.
