Job Description:
Purpose of Job: Improves the overall health of the communities we serve by delivering exceptional patient scheduling and supporting real‑time in-clinic care team coordination as follows:
Essential Duties and Responsibilities:
Provides excellent customer service as a first point of contact for patients, determining caller needs, managing calls appropriately, and delivering accurate information about services, locations, providers, and access options.
Schedules follow-up appointments before the patient leaves the clinic and, per our Team-Based Care model, provides in-room scheduling when appropriate to support continuity of care, scheduling accuracy, and enhanced patient experience.
Conducts proactive outreach to reschedule missed appointments and actively manages a patient waitlist to improve access.
Schedules visits in alignment with provider preferences and updated guidelines, ensuring accurate visit types, lengths, interpreter needs, and appropriate patient flow.
Monitors provider schedules for gaps, cancellations, and optimization opportunities and collaborates with onsite care team members to fill openings and adjust schedules based on workflow needs.
Develops and maintains expertise in clinic workflows, provider templates, and scheduling standards to support efficient operations.
Communicates appointment details clearly and ensures patient understanding of next steps.
Documents scheduling activity and patient communication actions in the electronic health record.
Supports scheduling and access-related quality improvement initiatives and updates scheduling practices in alignment with new guidelines or provider changes.
Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: High school diploma or equivalent required. Prior experience working in clinical setting strongly preferred. Prior customer service experience strongly preferred.
Skills: Attention to detail required. Knowledge of HIPAA regulations required. Excellent customer service skills required. Strong oral and written communications skills required. Compassion for patients required. Ability to function well in team environment required. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs, required. Computer skills required. Valid drivers’ license and insurance required. Commitment to supporting a safe and respectful environment is required.
Physical Demands: Must be able to move around the facility between one-third and two-thirds of the day, as well as sit or stand at a workstation and in patient rooms. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Communicating in person with patients and staff in clinical settings is required over two-thirds of the day, while climbing, stooping, or crawling is minimal. Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. occurs only occasionally. Must be able to travel between clinic sites to meet coverage needed and move about patient care areas.
