James River Home Health is actively seeking a compassionate and dedicated Hospice RN Case Manager to support our patients in the Roanoke and surrounding areas. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.
Salary Range: $67,700 - $97,600

We personalize our services to meet your situation respectfully, efficiently, and compassionately, fostering independence, preserving dignity and improving quality of life. The Agency is dedicated to providing the most qualified personnel to the patient in his/her own home in order to maximize and fulfill his/her goals as a member of the community. Services are available 24 hours a day, seven days a week using an answering service and on-call procedures after normal working hours, weekends, and holidays. As needed, staff may be assigned to make patient visits after normal business hours. We offer skilled nursing services and to the extent that appropriate professional staff are either hired or contracted.
