Location: Long Beach Northeast and Southeast.
About the Role
We’re seeking a compassionate Medical Social Worker to assess patient needs, coordinate community resources, and support in‑home care plans. You’ll help patients and families navigate care options, advance directives, and support services — and complete all documentation electronically right from the patient’s home.
What You’ll Do
- Assess social and emotional needs related to the patient’s health
- Support physicians and clinical team members with psychosocial insights
- Participate in care planning (Plan of Care / POC)
- Observe, document, and report community resource needs
- Educate patients and families on available community services
- Provide counseling and guidance to patients and caregivers
- Utilize and coordinate appropriate community resources
- Assist with discharge planning
- Participate in in‑service training and educational programs
Note: Duties represent core responsibilities but may include other related tasks as needed.
What You Need (Qualifications)
- Master’s degree from a Council on Social Work Education–accredited program
- Minimum 1 year of MSW experience
- Strong physical stamina and mental health
- Pre‑employment health clearance (TB test/CXR as required by state)
- Able to read, follow instructions, and accurately document care
- Self‑directed; able to work independently
- Current CPR certification
- Professional, patient, and flexible
- Excellent personal hygiene and communication skills
- Valid driver’s license and insured vehicle per Lorian Health requirements
- Able to verbally communicate in the patient’s language and document all care

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $75 to $90 visit commensurate with experience and depending on type of visit.
