Assess patients' social and emotional needs to coordinate community resources and support in-home care plans. Collaborate with clinical teams to provide psychosocial insights and assist with discharge planning and patient education.
Requires a Master's degree from a CSWE-accredited program and at least one year of MSW experience. Candidates must have a valid driver's license, insured vehicle, and current CPR certification.

Location: Long Beach Northeast and Southeast.
We’re seeking a compassionate Medical Social Worker to assess patient needs, coordinate community resources, and support in‑home care plans. You’ll help patients and families navigate care options, advance directives, and support services — and complete all documentation electronically right from the patient’s home.
Note: Duties represent core responsibilities but may include other related tasks as needed.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $75 to $90 visit commensurate with experience and depending on type of visit.
