The Physical Therapist will perform home health visits to conduct diagnostic tests, develop individualized care plans, and implement therapeutic interventions for adult and geriatric clients. They are also responsible for educating clients and families on health and safety while maintaining accurate documentation via electronic medical records.
Candidates must hold a current North Carolina Physical Therapist license and have at least one year of recent professional experience. A degree from an accredited physical therapy program is required, along with strong organizational and computer skills.

This position services adult and geriatric clients on a per visit basis in senior living communities throughout Catawba County in North Carolina.
As a home health Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
