The Patient Navigator will develop and implement marketing strategies to promote home health and hospice services while building relationships with physicians and community organizations. They will also collaborate with the clinical team to ensure marketing alignment and maintain accurate records of referrals in the CRM system.
Candidates should have a bachelor's degree in marketing or a related field and at least 3 years of experience in healthcare marketing or sales. Strong communication skills and a solid understanding of the home health and hospice regulatory environment are required.

The Home Health and Hospice Patient Navigator will promote our home health and hospice services and build strong relationships with referral sources. The Patient Navigator will have a passion for healthcare and a commitment to providing high-quality care to patients, families and their caregivers in their homes.
Schedule: Full-Time, Weekdays, 8 Hour Shifts, Day Shifts
