The Care Coordinator provides family-centered, community-based wraparound services to children with complex behavioral health needs. Responsibilities include developing individualized care plans, facilitating team meetings, and connecting families with community resources.
Candidates must hold a bachelor's degree in Social Work, Counseling, Psychology, or a related field. A minimum of three years of experience working with children and families in complex systems is required.
OhioRISE Care Coordinator
The OhioRISE Care Coordinator provides family-centered, community-based care coordination and high-fidelity wraparound services to children and families with complex behavioral health and system-of-care needs.
In this role, you will partner with children, families, and their support networks to identify strengths and needs, develop individualized care plans, coordinate services, and build a team of providers and natural supports around each child. Care Coordinators develop crisis and transition plans, conduct ongoing assessments, and help families successfully navigate multiple systems of care.
This position is ideal for a compassionate, culturally responsive professional who enjoys building relationships, coordinating complex services, and helping children and families access the resources they need to thrive.

Pay: Starting at $45,000/year, based on qualifications and experience.

Work Environment
This is a community-based position with services provided across office, school, neighborhood, and home environments. The role may involve working with children experiencing behavioral and emotional challenges and may include exposure to shouting or verbally or physically aggressive behaviors.
Employees may occasionally work in unclean environments or unfamiliar areas of the community and should be comfortable traveling throughout the service area.
Physical Demands

The position regularly requires sitting, standing, walking, bending, and lifting up to 50 pounds. The employee must also be able to safely operate a motor vehicle in changing weather and community conditions.
