We are looking for a passionate and experienced individual to become the Practice Manager on the Transitions Team to lead and oversee the delivery of Adult Social Care services.
In this key leadership role, you will be responsible for the day-to-day management of services within your area, ensuring the delivery of safe, effective, and person-centred support to adults and young people with learning disabilities.
You will drive performance, quality, and continuous improvement while supporting and developing a dedicated team.
This role may require travel within the service area and participation in out-of-hours support arrangements.
We are looking for candidates who have:
Relevant professional and/or management qualification.
Understanding of health and social care legislation.
Ability to make decisions when dealing with often constant and conflicting demands on time.
Knowledge of Safeguarding Adults at Risk procedures.
