Provide nursing care, treatments, and medication administration to residents according to medical care plans. Monitor resident health status, document progress, and coordinate with multidisciplinary teams to ensure quality care.
Must possess a valid NYS LPN License and pass a background investigation and physical exam. Preferred qualifications include experience in long-term care, EMR usage, and working with adult or aging populations.

NYS LPN License.
Preferred Qualifications:
One year of healthcare experience working with an adult or aging population in a clinical setting. One year of long term care experience. Experience using EMR.

Working Conditions:
Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.
