The Care Coordinator serves as an on-site liaison at a dialysis clinic to manage patient care plans, schedule provider visits, and ensure seamless communication between patients, families, and the clinical team. Ennoble Care is seeking a full-time Care Coordinator, Embedded @ Partnered-Dialysis Clinics in the Lehigh, Pennsylvania area. This on-site role serves as a key liaison within a partnered dialysis facility, ensuring coordinated, patient-centered care through structured coordination, strong clinical collaboration, and proactive communication.
This Care Coordinator works directly within one of our partnered-dialysis clinics to engage patients in person, coordinate care plans in collaboration with Ennoble providers and facility staff, schedule and organize provider visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team. This position plays a central role in maintaining continuity of care by monitoring patient needs, communicating changes in condition, addressing barriers to care, and maintaining accurate documentation to keep the care team aligned.

Ennoble Care provides home-based primary care, palliative care, and hospice for seniors who are frail, homebound, or close to it. We meet patients wherever they live, including private homes, senior communities, and long-term facilities. Our goal is to bring care to the places where it is needed most. Our clinicians visit patients many times a year, supported by daily care coordination, close observation of changes in a patient’s condition, and remote monitoring. That level of presence keeps small problems from becoming crises. As our Quality and Outcomes Report shows, hospital admissions fall 25 percent in the first year and 63 percent by the second year. Medicare hospital spend drops to roughly half that of comparable homebound populations. We serve approximately 50,000 patients across 15 states and the District of Columbia. Many live with multiple chronic conditions, cognitive impairment, and social isolation. Our work requires proximity, consistency, and a team that knows its patients well.
