The Medical Social Worker assesses patients' psychosocial, financial, and environmental needs to develop and implement a comprehensive Plan of Care. They coordinate with physicians and multidisciplinary teams to ensure quality continuity of care and effective discharge planning.
Candidates must hold a Master's or Doctoral degree in Social Work from a CSWE-accredited school and possess a current state license. At least one year of healthcare social work experience and a valid driver's license are required.
