The Nurse Practitioner provides primary care to enrolled seniors by evaluating physical complaints, performing examinations, and formulating treatment plans. They manage acute and long-term medical needs while collaborating with an interdisciplinary team in various clinical and home settings.
Candidates must hold a current State Nurse Practitioner license, DEA Certification with full prescriptive authority, and have at least 5 years of experience as a Registered Nurse in elderly care. At least one year of experience as a practicing Nurse Practitioner is required.

The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties.
