Take Another Chance is seeking a Direct Support Professional (DSP) to join our team.
The ideal candidate is reliable, team-oriented, and committed to providing high-quality, person-centered supports that help individuals live safe, fulfilling, and independent lives. This role includes assisting with activities of daily living, medication administration, transportation, community integration, and the implementation of individualized support plans while building meaningful relationships with the individuals we serve in a 24-hour awake care residential setting.
No prior DSP experience is required—we provide paid training and certification support for qualified candidates. Compensation:$16.00–$18.00/hour, with starting pay determined based on relevant experience, certifications, transferable skills, and job-related qualifications.
This is a part-time position with scheduling based on client needs and service requirements.
Key Responsibilities
Personal Care & Daily Living Support
Provide person-centered support with activities of daily living, including bathing, grooming, toileting, dressing, and meal preparation.
Assist individuals with mobility, transfers, and physical support needs while promoting dignity and independence.
Encourage skill development and increased independence through daily routines and individualized supports.
Health, Safety & Medication Support
Administer medications in accordance with agency policies and Ohio DODD requirements after completing required training and certifications.
Observe and report changes in health, behavior, or overall well-being to appropriate team members.
Follow Individual Service Plans (ISPs), behavior support strategies, and safety plans to ensure quality care.
Home & Community Integration
Support individuals in participating in community activities, appointments, recreational opportunities, and personal interests.
Provide safe and reliable transportation as needed.
Help maintain a clean, organized, and safe living environment.
Documentation, Communication & Compliance
Complete required documentation, including daily notes, medication records, and incident reports.
Follow company policies, ISP requirements, DODD regulations, and applicable standards.
Participate in trainings, team meetings, and shift communication to support continuity of care.
Relationship Building & Team Collaboration
Build positive, professional relationships with individuals served, families, guardians, and team members.
Promote an environment of respect, dignity, choice, and self-advocacy.
Work collaboratively with coworkers to provide consistent, high-quality support across all shifts.
Requirements
Must be at least 18 years old.
High school diploma or equivalent.
Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles.
Reliable daily access to a personal vehicle to transport clients.
Proof of active auto insurance meeting Ohio minimum requirements.
Ability to pass both a criminal and a driving history background checks.
Ability to lift up to 50 pounds and perform physical tasks, including bending, reaching, and standing for extended periods.
Willingness to complete required training and certifications provided by the company.

Preferred Qualifications:
Previous experience in direct support, caregiving, healthcare, education, customer service, childcare, or a related field.
Current AHA Heartsaver First Aid/CPR/AED Certification.
Current Ohio DODD Category I Medication Administration Certification or higher
Experience supporting individuals with developmental disabilities.
Experience with electronic documentation systems and Microsoft Office.
