Are you ready to build something extraordinary? Hospice of the Piedmont is seeking a visionary leader to establish and grow our pediatric hospice and palliative care program. This unique opportunity combines strategic leadership, clinical excellence, and the chance to shape the future of pediatric serious illness care in our community.
Qualifications:
Licensed as a Nurse Practitioner in the Commonwealth of Virginia
Current Nurse Practitioner certification from an accredited certifying body by a national body (PNCB, ANCC, NBCHPN, AANP)
Advanced hospice and palliative nursing certification, such as CHPPN, strongly preferred.
Minimum of ten years of progressive pediatric healthcare experience, including advanced clinical practice with children experiencing serious, complex, chronic, or life-limiting illness.
Demonstrated success in service line growth and strategic planning; designing, implementing, evaluating, and scaling new clinical programs or service lines, including stakeholder alignment, change management, and measurable outcome improvement.
Preferred experience in pediatric hospice, pediatric palliative care, complex chronic illness, or serious illness care for children and families.
Demonstrated competency in budget management and working with grants, foundations, and philanthropic partners.
Demonstrated ability to build trusted partnerships across health systems, children's hospitals, medical practices, hospice organizations, payers, community agencies, and philanthropic stakeholders.
Responsibilities/General Duties (Including but not limited to):
Develops, communicates, and executes a multi-year strategic plan for pediatric palliative and hospice care aligned with Hospice of the Piedmont's mission, organizational priorities, and regional community needs
Provides clinical care and coordinates services for pediatric patients and their families
Directs the design and development of the pediatric palliative care service line, including workflows, staffing plans, quality measures, community partnerships, and education for internal and external stakeholders.
Collaborates with referral sources and primary care providers to support smooth transitions and quality services for patients, families, and providers.
Serves as the principal pediatric liaison for external clinical and organizational partnerships.
Leads pediatric community-engagement and outreach to increase awareness, reduce barriers to referral, strengthen trust, and expand equitable access to hospice and palliative care
Works with other members of HOP staff to facilitate and document clarification of patient and family goals of care; partners with the Pediatric and Family Grief Counselor to develop and deliver a coordinated continuum of emotional, psychosocial, and grief support services for pediatric patients and their families.
Establishes and monitors a pediatric quality, safety, equity, and outcomes framework, including access, timeliness, symptom burden, goal-concordant care, care transitions, family experience, staff competency, utilization, and program sustainability.
Participates in community education and support
Collaborates with attending physicians, primary and specialty clinicians, hospice physicians, and the interdisciplinary group to develop, implement, and revise individualized plans of care that reflect clinical needs and patient and family goals.
Prepares and maintains accurate patient records, charts, and documents to support the medical practice and reimbursement for services provided in a timely manner
Leads the development, implementation, and periodic review of pediatric clinical standards, policies, pathways, protocols, and evidence-based practice guidelines.
Collaborates with key stakeholders to provide services and transitions for patients to hospice services
Consults with providers and key stakeholders as needed; collaborates with the Hospice of the Piedmont interdisciplinary group and other physicians; and informs the primary physician of recommended services or changes to the patient's plan of care.
Attends team lDG as appropriate, collaborates on appropriate plan of care for pediatric patients, integrates clinical assessment findings of face-to-face visit into recertification and team discussion, and provides educational support to IDG
Develops and provides education, training and consultation to colleagues, facility staff and community partners to enhance knowledge and support pediatric patients and families.
Adheres to confidentiality regarding patients, families, staff, and the organization
Assists with identifying and recruiting for coverage needs, participates in any on-call responsibilities
Upholds the standards of ethical behavior and professionalism in all conduct
Typical Physical Demands/Working Conditions:
Work may occur in office, clinical, patient home, or community settings
Able to push, pull, or lift 50 pounds, push up to 200 pounds in a wheelchair
It may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
Communicate by phone, writing, typing, speaking
Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs
While performing the duties of the job, the employee is regularly required to use hands to handle or feel objects, reach, kneel, walk, stand, balance and talk to hear.
